Comprehensive Diagnostic Screening

Our Comprehensive Diagnostic Screening is the first step toward resolving pain, improving mobility, and getting back to the activities you love.

During this 60-minute session, we will:

-Have a focused conversation about your symptoms, how they’re impacting your daily life, and what goals you want to achieve

-Perform a series of targeted movement and orthopedic tests to identify the root cause of your issue

-Outline a personalized treatment plan to help you move better, feel stronger, and stay pain-free

Whether you're dealing with chronic pain, a recent injury, or nagging limitations, this screening helps you get clear answers and a plan of action.

Performance-Driven Physical Therapy Treatment

Our physical therapy sessions are designed to help you build strength, restore function, and get back to the workouts or active lifestyle you love—without shortcuts or cookie-cutter programs.

Be ready to move—this isn’t a passive experience. It’s performance therapy with purpose.

We never double-book. Every session is private and personalized, so you get 100% of our attention.

Your Hybrid or In-Person Treatment Plan is created collaboratively during your Comprehensive Diagnostic Screening. Most plans include 4–12 sessions and come with Virtual Exercise Programming to support you between visits.

During each 1-on-1, 60-minute session, you can expect:

  • Personalized exercise progressions based on your current mobility, strength, and goals

  • Ongoing strength and movement assessments to track progress and adapt your plan

  • Hands-on manual therapy as needed to reduce pain, improve range of motion, and support recovery

  • A true workout-style rehab session that may include strength training, plyometrics, and cardio—because physical therapy should prepare you for real life and real fitness

Customized Exercise Programming for Rehab & Performance

This comprehensive program is included as part of your Hybrid Physical Therapy or Virtual Coaching experience—designed to support your recovery, boost performance, and help you reach your goals faster and more effectively.

After the Diagnostic Screening, our focus is on strength, mobility and form to improve your performance at specific movements.

$180/hr in person or $250/month virtual programming.

Virtual Fitness Coaching

A comprehensive Coaching program that can be Hybrid or completely Virtual via app based Exercise Programming. We focus on building healthy habits so that your fitness journey can be sustainable for life.

  • Customized Exercise Programming that can be done either in person or on your own time

  • Nutrition Guidance and support

  • Strength training and Cardio

  • Weekly check-ins

  • Weekly feedback from your Coach

  • Changes made to your program based on your schedule and preferences

$250 per month, add on in-person sessions for $79/hr

Medical Fitness

Our Medical Fitness Program is designed for individuals who want to improve their health through exercise but need extra guidance and monitoring due to medical conditions or health concerns.

Led by a licensed Doctor of Physical Therapy, this program ensures you’re exercising safely, effectively, and with a plan that aligns with your medical needs.

This is more than just personal training—it’s health-anchored fitness, built for your unique needs.

Ideal for individuals with:

Frequently Asked Questions

Do I need a referral?

No! Florida is a direct access state which allows you to work with a Doctor of Physical Therapy for 30 days without a prescription or referral from a Physician. If you still need treatment after that, we take care of all the paperwork! If you already have a referral, we can use it and simply need a copy for our records.

Do you take insurance?

We do not accept insurance. We believe that your care should not be limited by the beliefs of a corporate insurance company. We strive to provide the highest quality care with our full attention, 1 on 1 appointments, and hybrid services. Unfortunately, taking insurance would not allow us to provide the care you deserve.

We can utilize your HSA/FSA funds or you are able to submit claims to your insurance company for Out of Network coverage to possibly be reimbursed for services provided.

What is Hybrid Physical Therapy?

Hybrid PT is a unique blend of in-person and digital services that utilizes a variety of forms of digital follow-up, which may include text messaging, phone calls, video calls, or app based exercise programs. At your appointment, Dr. Farmer will determine the combination of services that will best help you achieve your goals.
